Lecture Summary

 Precision oncotherapy demands more than tumour killing, it requires programmable selectivity that confines toxicity to malignant tissue while actively protecting organs vulnerable to treatment-related injury. This talk introduces the concept of “intelligent stem cells” as autonomous, logic-gated therapeutic platforms that couple tumour recognition to a predefined therapeutic output, enabling a shift from conventional “aggressive enrichment” strategies toward a defensive precision paradigm. At the core of this intelligence is a reconstructed Notch signalling architecture in which the extracellular recognition domain is redesigned to bind tumour antigens (e.g., Her2), while the intracellular module is replaced with synthetic transcriptional activators that trigger bespoke gene programs only upon contact-dependent receptor engagement. This physical-contact requirement functions as a biological “lock,” keeping engineered mesenchymal stem cells (MSCs) inert during transit through healthy organs. Two delivery modes exemplify this precision: (i) intelligent nanoenzyme carriers, where MSCs preloaded with Cu/Fe nanocrystal clusters initiate HSV-TK expression upon tumour contact and release their payload via ganciclovir-controlled rupture; and (ii) intelligent drug factories, where engineered MSCs synthesise and secrete anti-angiogenic biologics (e.g., endostatin) only at the lesion site, reducing off-target exposure and mitigating systemic complications such as cardiotoxicity. Precision oncotherapy must also preserve long-term quality of life by repairing collateral tissue damage. Here, MSC-derived extracellular vesicles (MSC-EVs) provide a complementary, cell-free reparative layer. Menstrual blood–derived MSC-EVs restore ovarian function after cisplatin injury by transferring HSPA8 to granulosa cells, upregulating MGARP, stabilising the antioxidant PRDX2, and reversing mitochondrial dysfunction and oxidative stress. Together, intelligent MSCs and MSC-EVs outline a dual strategy—tumour-locked therapy plus organ-preserving repair toward safer, truly precise oncotherapy.
